outgoing filtering

I have a really weird one, irf I go and look at the quarantine folder, i see a mail that has been quarantined from User A, who is a user on our system, If I look in the smtp log file I see the mail is received and no issues are seen. If I look at the delivery log I see the mail being sent From User A to user B, no spam issues seen and the mail is delivered to the remote server, yet the mail exist in the quarantine folder.

My outgoing spam score is set to 35 to trigger a mail quarantine. So now I dont know if the mail in fact was delivered or was taken and moved to the quarantine folder and not delivered to  User B. 

Can someone help me with this please..
