Thought that the reset bug .. would have gone away?

We have written a new process that reads the ADMIN log and take action on logins that to a new IP. (just finished ) ... soooo we see this:
06:30:52.243 [162.243.242.176] user @ successfully force-reset-password

Was surprise to see this.... thought that it was hack that was identified and coded around.
The current email system is 100.0.9575.20827 (3/20/2026)
