Add Display Name to new account form.

Proposed

It would be a benefit to us and reduce mouse swipes and keystrokes if we could fill in the Display Name when creating a new account rather than having it default to the email address.

+1 Name and email address make sense. The rest is gibberish that can be filled out later.
