Question on Gateway Usage (Possible Problem???)

We are on the latest MAPI beta. We have every domain set to use a custom gateway. So why would any email leaving our system be using the server IP address for outbound? I cannot see why. None of the gateways are round-robin. They are set to specified domain and I have confirmed 100% are using a gateway.

This just came across my email. We get this, then it goes away, then we get it again.
