New Ticket Form Will Not Close When Inserting Form

When I create a new ticket and use the "Insert Form" menu option to insert one of my forms, the ticket does get created but the window does not close.  More importantly, clicking on the "Complete Form" link will bring you to the web site and gives you a message saying "The form is no longer available".  
However, the "Send Form" menu item works properly when inserting a form into an existing ticket.
Please take a look at this.

Hi Terry,

Thanks for reporting this issue. It has been resolved and the fix will be included in the next minor update of SmarterTrack 14.x.
