Event - Ticket Idle Doesn't Fire

Event Ticket Idle
If Idle greater than 2 minutes
If custom field doesn't equal "VALUE"
 
Send email alert.
 
The event doesn't fire, no email is ever sent.

The system only fires the idle events if a ticket is sitting in the system queue when it goes idle. Regardless of what we set in the event settings this is the behavior we see.
