Why SPAM Filtering didn't work?

Would have expected the SM to mark this as either SPAM HIGH or SPAM LOW .. but it didnt mark either.
For our part ... this came from inbound server XX.XX.XX.123 ---> to Main Mail server

What other configurations needs to be setup?
  On the main mail server... under security ..   XX.XX.XX.123  has the following

If the inbound server x.x.x.123 is an SMTP gateway, and you are exempting it from spam checks, the mail will be delivered no matter the score. Am I missing something here?

This header extract says that Declude should have passed a result of 38 to Smartermail.    You need to check the Smartermail spam checks log to see what happened next.  Declude log and Delivery lig may have useful data as well
