Email not in sent items or sent messages

A customer has told me they replied to an email from webmail today, CC'd a co-worker and the co-worker got the email and the recipient also but the said message did not appear in any of the sent folders.
It wasn't forwarded - Any reason why this might have happened?

eems to be random
I can see the messages have been sent in the logs, but customer worries that the only way they know the message has been sent is if they get a reply?
this is still an urgent issue
