Off the shelf and out of the box Smartermail cannot do self-registration of new users.

You can certainly automate this using Smartermail's Web Services (see http://mail.smartertools.com/services/) to code your own solution. Alternately you can setup and configure WebsitePanel (http://www.websitepanel.net/) with Smartermail to do this for you.
