Blocked and trusted sender options not available!

Is anyone else seeing varying options available for Block Sender / Trust Sender?

My client is seeing different to me on his account (when I impersonate).
He uses "Block Sender" quite a bit and he's now saying it says "unblock sender" for all emails.

If I right mouse click an email in the list I see "Trust Sender" greyed out, so i can't select.
And "Unblock Sender" when I know the sender isn't blocked. Even when I select this to unblock, it's still the same.

This is the same email in my account (my clients is different.
Firstly right mouse clicked in the email in the list:


I get "Trust Sender" greyed out and "Unblock Sender", this sender isn't blocked.

Then clicked on the 3 dots to the right of the "Mark" dropdown:


As you can see, 2 different sets of options for the same email. I get "Trust Sender" and "Block Sender"

My client doesn't get the "Block Sender" at all.
Any ideas?

Surely both of these drop downs should display the same features in the same state as it's the same email?
Anyone else seeing this or am i going mad?
