Failover Monitoring for SmarterMail

We are considering using SmarterMail with automatic failover, but I cannot get a clear answer on what criteria I should monitor to decide when to initiate a failover between servers. For people who have set this up before, what did you monitor? 

I've been told obvious stuff like watching port 25, the depth of the mail queue,  and using ping, as well as more complex like looking at IO wait time. I'm just not sure where to go.

Gary, my personal recommendation on this is to leverage an SMTP based sensor (such as the one found in PRTG) to periodically connect, authenticate, and perhaps even send a test message. If any part of this process fails you can typically assume the SmarterMail service has stopped or the server is otherwise unavailable.
