Outlook Addresses going to Wrong Address

Anybody ever seen this?  We have six domains using latest SM v15 and have had this issue for YEARS!  Every now and again SmarterMail will pick on a particular user and a stored address.  So for example the user picks John.Doe@gmail and sends an email, but the wrong person receives it.  Even shows the wrong person in Sent Items instead of the address you picked.  So example instead of going to John.Doe@gmail it goes to Sally.Johnson@yahoo or whatever.  Only way to correct this is delete the stored address and retype it.
That's not a very safe thing to do...suggestions?

There are innumerable posts about Outlook and issues with the NK2 file, and these issues have been around for a very long time. It's not a SmarterMail issue as much as it is an Outlook issue.
