SMTP Blocking Rules not being enforced

One of my users reported getting viruses in his inbox.  Not only did ClamAV not detect the virus twice, but a rule under SMTP Blocking did not stop it either when it should have.  
 
I have setup a rule under SMTP Blocking to block Email Address [incoming] for *.vn 
But that rule did not stop it.
The email was coming from 
Sears.69@static.vnpt.vn
I have pretty much taken the nuclear approach to these new Top Level Domains, and have a rule to block most of them.
 
Any reason this email would get through even with this rule in place?

Make certain that the user does not have custom spam or routing options enable.

Ih he does, disable them, go into his account, and clear the roting rules. They wreck havoc with antispam control, and we've blocked thier use completely - no exceptions.
